Referral packets arrive as large, unstructured documents, which can hide critical clinical deal‑breakers, delay communications with referral sources, and contribute to staff burnout and revenue loss. ExaCare AI aims to transform this by enabling rapid, confident decisions the moment a referral is received.
The AI platform ingests unstructured packets, surfaces clinical deal-breakers, highlights risk and readiness signals, standardizes criteria across teams, and streamlines communication to reduce back-and-forth. The goal is safer, faster placements with less administrative burden, empowering care teams to focus on people and outcomes.
